In big hospital settings, most of the time the dr is basically a contractor of the hospital. Kind of like a barber who rents a chair. They charge their own fees and then reimburse the hospital based on whatever contract they figured out. The contracts vary greatly so the situation will be different from place to place.
Even hospital docs are for the most part still proprietors. They just reimburse the hospital for use of the equipment and technology, and of course for the stream of patients hospitals have coming in.

I think it just exemplifies the point that the health care system is so fricking complicated and the 'solution' may not even exist. I know a fair amount about Obama care but will readily admit I don't understand much of the implications throughout the whole healthcare process. It's a mess.
Average joe American really has no idea what it is even about. It is so sensationalized by the media due to the political side of it, that it is impossible to get any actual information it seems.

Good luck with your case against the folks that insure Kaiser Vic-hopefully they give you a bunch of $$ just to go away. We all deserve great health care-but I am not seeing that at all. Just try to stay healthy and don't trust doctors- or insurance comapnies unless u are forced. Luck to all-we are going to need it-OD

Seems like giving me money to go away is what is most important to them. They are almost too willing to just how me the 250k which is the cap in Cali. I will admit I don't no half of what I really should but from what I've had explained to me at union meetings and rallies and what not they really want me to believe Obama care is good. Like u said though can't trust media can't trust docs or insurance I guess just gotta trust the ol gutsigpic

couple of points...
1) health care is currently at 17% gdp, and we have approximately 40 million without coverage of any kind.. we have higher infant mortality, shorter life spans compared to England where they are complaining about 100% coverage and 9.3% gdp.. given this ..I think there can be dramatic savings and improved coverage .gdp in the US is such a huge number that the amount we are paying for coverage is absolutely over the top.. and these uncovered people is rationing !!!!
2) it is not.. repeat not Socialised medicine..the profit motive is still there to deny you coverage..
healthcare has never followed the laws of supply and demand, so if nature doesn't provide the law, man does need to.the vast majority will end up in state sponsored private pools ...but mandated by law
3) working poor have no coverage, so if they get sick , they stay sick or go the last step to welfare
and disability ...if it is a chronic problem they stay there ..
I know someone with kidney transplant while on ss... she will be on expensive drugs the rest of her life.. so what incentive is there to get a job?, well luckily the state realised this in regards to the punishment to get a job, so they allow her a certain amount and pick up the cost for the kidney drugs..
but this is only for transplants ( and transplants are cheaper than dialysis in the long run)
4) how much is spent on guaranteeing you have coverage before the hospital does anything?
5)I saw Simpson and Bowles this week at a speaking engagement , they took questions from the audience and basically dismiss everyone that was talking about obamacare saying this is not the problem. ( I was surprised to see Simpson actually take on a veteran and point out that his $500 he was paying for health care was nothing compared to the $5600 it was worth .. and said that was an entitlement too..) Obamacare is a joke compared to defense , ss, medicaid and medicare
Old people are expensive , and the amount the average retired person paid into the system is spent in the first year retired ..I am not picking on Old people , but we need to move the retirement age..
defense we spend more than the next 17 countries combined including some of allies
6) the debt limit is not the place to have this debate.., the basis for the value of the dollar is that it is the currency that will be there tomorrow .. think of it this way the US has yet to default on it's debt.. and the government has the right to print to cover it ..so until now you know it is there ... say you live in Egypt or some country in a crisis, and you have a bill due in a week... you have the cash now and want to earn a little interest.. where can you put it an know it will be there in a week.. Until now the US has been the answer .. and there fore the US treasury bill interest rate is what everything else in the world is based on... we loose that and interest rates in the US will go up, raising the debt without spending a dime because of the increased treasury rates... The congress is playing with fire and judging be a recent poll, I am a little scared that the average american has no clue..

Don't need to move the retirement age-need to get older amaericans to be more healthy !! I am at an age where I am simply sick of a bunch of older fat fucks clogging up the system. They for the most part are NOT sick-they are FAT n Lazy.They eat like shit-no exercise and take no good sups. Just crap from the doctor where they go weekly when is not needed. So-if you move the age-thse fat asses just go on disability ?? Very good points in post though xdude-Thanks-OD

well in regards to older people rehabing.... that is a good thing...
rehab cost a lot... but the alternative is nursing homes... you want to see money go fast ..look at a nursing home.. count each patient and multiply by about 300-500 a day .. that is on the low end 100k -200 k a year ..
I used to work on Scanning equipment.. (xray,ct,mri) and I will tell you that stuff is all just cover your ass medicine .. the doc knows when you have a torn Achilles but they order an MRI ..why cause insurance will pay for it , doctors own the machine and even though there is a .1% chance it is not the Achilles , they have done the procedure to prove it 100%
your health care dollar should be spent on whipping you into shape...not on useless tests..
When I was leaving they came up with a mri scan to prove you had gall stones, why cause they could..
it cost a lot of money and was not really needed..
to argue that cost do get some control , I recently went for a ultra sound ,wow, that technology has come along in a huge way to being something cheap and detailed .. I was amazed at the detail..
in England the front line doctors make more money then in the US and are paid bonuses on keeping their patients health ( ideal weight , stop smoking vaccinations etc) in the US they are paid for follow ups visit .. sure you can't get that bone marrow transplant in 24 hours,or an MRI appointment that day but chances are better you won't need it ..
I am just saying this system in the US is not the best in the world as people claim... It is best on the extreme , but fails miserably at the basics..(way off subject but has anyone seem that show on the discover health about the guy with a 132 lb testicle.. he didn't have insurance and let the growing problem grow ( literally) until he got a doctor to volunteer ..Of course the doctor was just looking for the fame , but it is an example of the basics..this should have been taken care a long time before it reach 132lbs IMHO)
Oh any one out there arguing about paying Death Counsellors I will stand up for the DC's. every person as they retire should have an appointment forced on them... why.. to know when you want to cut the cord.. I recently lost my mom and all I knew about her wishes was she did not want to be hooked to a machine.. but there is the rub... define What is a machine ..a resperator , a heart stimulator , a feeding tub? when do you say, they are ready to let go? see what I mean.. US medicine , if there is no DNR or medical proxy , must assume that you want the max.. I don't know how many times I was at the hospital and they were giving an MRI to a patient that was "not responding" there was nothing wrong they were just dying , let them go and stop billing insurance and the government ( ss , medicaid medicare etc) We had a nurse that acted as a counsellor and made what we thought was the right choice, based on our mother .. but if it was in writing when to let go then their wishes would be known..
I am not saying kill old people , just let them die with respect... if you get 5 months more with out regaining consciousness then what is the point ..in the end my mom did not wake and we provided pain killers to keep her comfortable...she did not go to the hospital , she died in a her bed at the nursing home.. going to the hospital would have been a huge waste of money...
the raising of retirement again was the easiest solution., and since most peoples 401K have been raided by Wall street , this may have happened any way.. ( don't get me started on that, I will just say it you are not managing your own 401K you are being robbed at brief case point ) SS was founded on an average retirement age 2 years after the life expectancy of 63...now it is 86 .. Simpson - Bowles was to raise it to 67, that 2 years would could be a huge cut back... instead congress ignored the problem..
